The UCCEDD Seeks Community Input on Five-Year Plan

The University of Cincinnati Center for Excellence in Developmental Disabilities (UCCEDD), will be submitting its new five-year plan to the Administration for Community Living (ACL) this fall. It is very important to us that we get input from community members on our goals and activities. We have created a brief survey where you can share your thoughts and experiences with us. Ultimately, your responses will meaningfully contribute to our plan for activities for the next five-years.

We kindly ask that you to complete the survey by

Friday, October 1, 2021. Your responses will be anonymous and we will not know who checked which answers.

Take the survey here.

If you have any problems with taking the survey or have any questions, please contact Jennifer Walsh, jennifer.walsh@cchmc.org

Thank you for taking our survey. We appreciate it.
