Clinic Appointments

Request for Clinic Appointments Now Open! Cincinnati Children’s Hospital (CCHMC) will be offering a limited number of clinic appointments in various specialties on Thursday, July 9, 2020 during the day at the CCHMC campus. Specialties available for appointments are: Epigenetics and Developmental Pediatrics, Appointments will be consultative visits related to RTS and will last somewhere between 20 – 45 minutes. If applicable, families may be able to utilize their own insurance. However, families will still be financially liable for all associated costs for clinic appointments and may be required to pay in full prior to or the day of their scheduled appointment.

Clinic appointment requests will be open until capacity is reached

Requests for clinic appointments is a two- step process.

1.) Complete the Online Clinic Request Form HERE

2.) After completing Step 1, fill out the Patient Information Form PDF

and email RTS@CCHMC.org

Both steps must be completed before request can be reviewed

Completing the required clinical appointment forms does not guarantee clinic appointments at CCHMC.
